FOLLOW THE PROCEDURES BELOW TO COMPLETE YOUR SERVICE SELECTION REQUEST.

This letter is required of all Navy Option midshipmen.

  1. Fill in all information listed in blue with what applies to you (use black to fill in your information).   Replace the blue remarks with your own comments.  Limit your remarks to keep this letter to one page.

    Fill in your designator choices in the blocks provided.  Your first through third choices must be unrestricted line community and your forth choice must be from the restricted line community.  If your first choice is 1160 you do not need to fill in your second and third choices.  If you desire to be considered for 1180 or 1190 this must be your first choice.

  2. Not later than 14 April 2008:  Print and sign the letter and turn it into the Admin Officer.  Last save the letter as your last name and e-mail it to your Company Advisor no later than 14 April 2008.  Use this link to open the Service Selection Request letter.
